Here is the process that will happen once you fill out the form below!
We will contact you by email and/or telephone.
We will discuss and Identify the QI studies of interest. If undecided, we can assist you with selecting study topics.
A proposal for services and an Independent Contractor Agreement will be provided for your review, signature, and return.
An invoice for the one-time registration fee commensurate to the selected studies will be sent to you.
Upon registration invoice payment, QI tools will be provided or developed in collaboration with you.
